What Is Apple Cider Vinegar?

Apple cider vinegar is made through a two-step fermentation process. First, crushed apples are exposed to yeast, which converts the fruit's natural sugars into alcohol. Researchers have identified several complementary mechanisms through which acetic acid may affect metabolism:

1. Slowing carbohydrate digestion and gastric emptying

Acetic acid inhibits salivary alpha-amylase and intestinal disaccharidases -- enzymes that break down starches and sugars -- reducing the rate at which glucose enters the bloodstream. The most comprehensive evidence to date comes from a 2025 systematic review and meta-analysis published in Nutrients, which pooled data from 10 randomized controlled trials involving 789 participants. The pooled analysis found that daily ACV consumption significantly reduced:5

Subgroup analyses showed that ACV reduced body weight and BMI in both overweight/obese individuals and in adults with type 2 diabetes, while reductions in waist circumference were significant only in the diabetic subgroup. Studies delivering 30 mL per day for up to 12 weeks drove the strongest signals. The authors described ACV as "a promising and accessible adjunctive strategy for short-term weight management" but noted the absence of long-term data.

The satiety data from Ostman et al. provide a plausible mechanism: a dose-dependent increase in satiety ratings after a vinegar-supplemented bread meal suggests that consistent pre-meal ACV may modestly reduce overall energy intake over time.6

Perspective

Effect sizes are modest. ACV is not a standalone weight-loss agent; the studies that showed the most benefit also incorporated reduced-calorie diets. It may add incremental support when paired with an overall healthy eating pattern, but expectations should be calibrated accordingly.

Cholesterol and Lipid Support

Several randomized trials have measured the impact of ACV on lipid panels, and a 2021 meta-analysis synthesized these findings.

What the research shows

The Hadi et al. (2021) meta-analysis of nine randomized clinical trials found that ACV consumption produced a statistically significant reduction in:4

The analysis found no significant effect on LDL-C, HDL-C, fasting insulin, or HOMA-IR in the pooled dataset. However, subgroup analysis revealed greater cholesterol and triglyceride reductions in people with type 2 diabetes, in those consuming 15 mL or less per day, and in studies lasting more than 8 weeks.

An 8-week RCT in type 2 diabetic patients corroborated these findings, reporting significant reductions in LDL cholesterol (p less than 0.001) and improvements in total-to-HDL cholesterol ratio (p=0.001) in the ACV group.3

Perspective

The lipid data are encouraging but not definitive. Effects on LDL in isolation are inconsistent across studies, and the clinical magnitude of total cholesterol reductions (roughly 6 mg/dL) is modest. ACV should be seen as a potential dietary adjunct, not a replacement for evidence-based lipid management strategies.

Safety, Side Effects, and Interactions

ACV is generally recognized as safe at culinary amounts, but concentrated supplemental use carries real risks that deserve careful attention.

Tooth enamel erosion

ACV is highly acidic (pH approximately 2-3). Drinking it undiluted or sipping it slowly over time exposes teeth to prolonged acid contact and can erode enamel. Always dilute ACV in at least 8 oz (240 mL) of water. Drinking through a straw and rinsing the mouth with plain water afterward are also advisable.

Esophageal and digestive irritation

Hypokalemia and bone health concerns
