Platelet-activating factor (PAF) antagonism and circulation

Cognitive Support in Aging Adults

This is the most extensively researched area for ginkgo biloba, and the evidence is strongest - though still modest - in adults who already have mild cognitive impairment (MCI) or early dementia.

What the research shows in MCI and dementia

Multiple independent meta-analyses of randomized controlled trials reach similar conclusions. A 2015 systematic review and meta-analysis of nine RCTs involving 2,561 patients found that EGb761 at 240 mg per day was able to stabilize or slow decline in cognition, activities of daily living (ADLs), neuropsychiatric behavior, and global clinical impression of change over 22-26 weeks compared with placebo.1 Neuropsychiatric symptoms - including agitation and anxiety - showed some of the most consistent benefits in that analysis.

A 2014 meta-analysis of seven placebo-controlled RCTs enrolling 2,684 patients found statistically significant improvements in cognition (standardized mean difference -0.52; 95% CI -0.98 to -0.05), daily living activities, and clinical global rating, with comparable rates of adverse events between EGb761 and placebo groups - indicating that tolerability was good.4

A larger 2016 meta-analysis covering 21 RCTs and 2,608 participants found that ginkgo biloba combined with standard care improved Mini-Mental State Examination (MMSE) scores by 2.39 points in Alzheimer's disease (MD 2.39; 95% CI 1.28-3.50; P<0.0001) and by 1.90 points in MCI (MD 1.90; 95% CI 1.41-2.39; P<0.00001) at 24 weeks. Activities of daily living and adverse event profiles also favored ginkgo biloba.3

An overview synthesizing 12 systematic reviews concluded that ginkgo biloba extract shows potentially beneficial effects for people with dementia when administered at doses above 200 mg per day for at least five months. Does ginkgo biloba work for cognitively healthy adults?

The controlled trial evidence here tells a clearly different story. A meta-analysis analyzing randomized trials involving 1,132 cognitively intact participants found near-zero and statistically non-significant effect sizes for memory (d = -0.04), executive function (d = -0.05), and attention (d = -0.08). These null findings held regardless of participant age, trial duration, or dose studied.2

Mood and Anxiety Support

A smaller body of clinical evidence suggests that ginkgo biloba extract may help support a calmer stress response in certain populations, though this area requires more research before strong conclusions are appropriate.

What the research shows

One well-designed double-blind, placebo-controlled RCT by Woelk and colleagues randomized 107 adults with generalized anxiety disorder (GAD) or adjustment disorder with anxious mood to either 480 mg per day EGb761, 240 mg per day EGb761, or placebo for four weeks. Both active treatment groups showed significant reductions in Hamilton Anxiety Rating Scale (HAMA) scores compared with placebo - with p=0.0003 for the higher-dose group and p=0.01 for the lower-dose group. A statistically significant dose-response trend (p=0.003) was also observed, suggesting the relationship was not a statistical artifact. The extract was described as safe and well tolerated across all groups.7

Consistent with these findings, meta-analyses of EGb761 in dementia populations also report reductions in neuropsychiatric symptoms including agitation and anxiety, which may reflect a shared mechanism.1

This evidence base is currently limited to a single anxiety-specific RCT of short duration. More research in larger populations and with longer follow-up is needed before this benefit can be considered well established for the general supplement consumer.

Tinnitus and Peripheral Circulation: An Honest Look

Two popular uses of ginkgo biloba - tinnitus relief and peripheral circulation support - are not well supported by current controlled trial evidence.

Tinnitus

Ginkgo biloba is frequently marketed for tinnitus (ringing in the ears), but the highest-quality synthesis of the evidence reaches a cautious conclusion. A 2022 Cochrane systematic review examined 12 randomized controlled trials involving 1,915 participants and found "little to no effect" on tinnitus symptom severity (Tinnitus Handicap Inventory scores) compared with placebo, with the overall quality of evidence rated as very uncertain across all measured outcomes.6 The review found no serious adverse events such as bleeding or seizures across the included studies. While some individual studies report positive effects in specific subgroups - particularly those with tinnitus associated with dementia or high anxiety - there is currently insufficient evidence to recommend ginkgo biloba for tinnitus as a primary complaint.

Peripheral artery disease and intermittent claudication

For intermittent claudication (leg cramping from reduced blood flow in peripheral artery disease), a 2013 Cochrane systematic review and meta-analysis of 14 RCTs involving 739 participants found no clinically meaningful benefit. The mean improvement in absolute claudication distance with ginkgo biloba over placebo was approximately 64.5 meters - a difference that did not reach statistical significance (P=0.06) - and the authors noted that publication bias likely inflated even this modest estimate.8

For both conditions, current evidence does not support ginkgo biloba as a first-line or established intervention.

Safety, Side Effects & Interactions

Ginkgo biloba has a generally acceptable safety profile at standard doses in most healthy adults, but several risks are clinically meaningful and require careful attention before starting supplementation.

Drug interactions - elevated bleeding risk

This is the most important safety concern. A large retrospective study using a Veterans Administration database found that concurrent use of ginkgo biloba with warfarin significantly increased the risk of a bleeding adverse event (hazard ratio 1.38; 95% CI 1.20-1.58; P<0.001).10 A 2025 analysis of 2,647 prescriptions found significant correlations between ginkgo biloba drug interactions and both bleeding risk (OR 1.08) and abnormal coagulation profiles (OR 1.49), with the highest risks seen when combined with antiplatelet agents such as clopidogrel and aspirin.11

  • Do not combine ginkgo biloba with warfarin, clopidogrel, aspirin (therapeutic doses), heparin, or other anticoagulant or antiplatelet agents without explicit physician guidance.

Raw ginkgo seeds are toxic - leaf extract only

Ginkgo seeds contain ginkgotoxin (4'-O-methylpyridoxine), which interferes with vitamin B6 metabolism and can cause seizures, loss of consciousness, and in high doses, death. Supplement use must be limited to standardized leaf extracts only. Never consume raw or processed ginkgo seeds as a supplement source.
