What the research shows

Randomized controlled trial (human): A 2020 RCT published in the Journal of Human Hypertension enrolled 143 adults meeting prehypertensive criteria under the 2017 ACC/AHA guideline and randomly assigned them to receive two 100 g servings (200 g total) of soursop fruit juice per day for 12 weeks, or no intervention. At both 6 and 12 weeks, the treatment group showed significantly lower systolic blood pressure and significantly lower serum uric acid levels compared to controls. Rates of progression to full hypertension were also lower in the soursop group. The authors concluded that soursop supplementation "can lower BP and SUA [serum uric acid] levels."1

The flavonoids and high potassium content in soursop fruit pulp are the most likely contributors to blood pressure modulation, consistent with known mechanisms of dietary polyphenols and mineral balance in vascular health.2

Important caveats

  • This is a single trial with 143 participants and no blinding; independent replication is needed before drawing strong conclusions.
  • The intervention was whole fruit juice, not a concentrated supplement; equivalence to commercial extracts cannot be assumed.
  • The Annona muricata pharmacological activities review (2022) identified antihypertensive effects as one of the nine documented activity categories across 49 studies, though most evidence was preclinical.11

Relaxation and sleep support

Traditional use of graviola leaf tea for insomnia and anxiety is supported by one preclinical mechanism: a 2018 animal study published in the Iranian Journal of Pharmaceutical Research found that hydroalcoholic leaf extract produced sedative effects at doses of 25-100 mg/kg and anxiolytic effects at 100 mg/kg in mice. Partial reversal with a benzodiazepine receptor antagonist (flumazenil) suggests interaction with GABAergic pathways. Quercetin and kaempferol were identified as likely active constituents.10 No human trials on sleep or anxiety have been conducted. These findings are Tier C and cannot be generalized to human use without clinical confirmation.

Safety, Side Effects, and Important Precautions

Graviola carries a more complex safety profile than many botanical supplements. Consumers should review these points carefully before use.

Overall safety at studied doses

A 2020 systematic review in the Journal of Pharmacy and Pharmacology examined safety data across in vitro, animal, and human studies and concluded that A. muricata leaf extract has "a favourable safety and tolerability profile" at studied doses. Human studies testing up to 300-540 mg of leaf extract daily reported no grade 3 or 4 adverse events.13 Animal toxicity studies found no adverse effects at doses up to 5,000 mg/kg in acute exposure models.6

Neurotoxicity risk from high or prolonged consumption

This is the most critical safety concern. Graviola - and related Annona species - contain annonacin, an acetogenin that inhibits mitochondrial Complex I (NADH:ubiquinone oxidoreductase). A landmark 2005 study in Movement Disorders measured annonacin content across different preparations:

  • A typical soursop fruit: approximately 15 mg of annonacin
  • A standard can of commercial soursop nectar: approximately 36 mg
  • A single cup of leaf infusion or decoction: approximately 140 micrograms

The authors estimated that consuming one fruit or one can of nectar daily would deliver, over one year, an amount of annonacin comparable to that which induced brain lesions in animal models. An epidemiological association between high, prolonged soursop consumption and an atypical form of parkinsonism observed in Guadeloupe has been reported in the scientific literature.12

Practical implication: Occasional consumption of the fruit as food is not in the same risk category as daily high-dose supplementation or regular leaf tea consumption over months. Standardized leaf extract supplements standardized for polyphenols (not for acetogenins) and used at recommended doses appear to carry lower risk, but long-term human safety data remain limited.

Contraindications

  • Pregnancy and breastfeeding: Insufficient human safety data. Avoid use.
  • Parkinson's disease or other movement disorders: The neurotoxicity mechanism described above is a direct contraindication.
  • Liver conditions: The same systematic review noted hepatoprotective effects at low doses but potential hepatotoxic signals at higher exposures; use with caution in individuals with liver disease.13

Drug interactions

  • Antihypertensive medications: Additive blood pressure-lowering effects are plausible given the human RCT data; monitor blood pressure closely if combining.1

The seeds of the soursop fruit contain concentrated toxic compounds and must never be consumed. Remove all seeds before eating the fruit.
