Respiratory Comfort and Cough Support
Supporting respiratory comfort is mullein leaf's most historically consistent use, and it is the indication most commonly marketed in modern dietary supplements. However, it is important to be transparent about the current state of the evidence.
What the research shows
As of the current literature, no standalone placebo-controlled human clinical trial has tested oral mullein leaf extract specifically for cough or respiratory outcomes. What does exist is a substantial body of ethnobotanical documentation and preclinical science.
Narrative and systematic reviews confirm that mullein has been used across at least 10 Verbascum species to treat respiratory pathologies in folk medicine traditions from Spain, Turkey, and beyond.2 A major 2005 review of common mullein's pharmacology noted its traditional use for pulmonary problems, asthma, and spasmodic coughs, and cataloged the biological plausibility of these uses based on the plant's chemical profile.1
The mechanistic rationale is reasonably well-established: saponins in the leaf may help thin mucus and support its clearance from the airways,6 and the anti-inflammatory flavonoids and verbascoside may address the underlying mucosal inflammation associated with respiratory discomfort.2,4

Ear Discomfort: The Strongest Human Evidence
The most robust human clinical evidence related to mullein leaf comes from two randomized controlled trials of a topical naturopathic ear drop formula containing mullein (verbascum) flowers, garlic, calendula, and hypericum in olive oil.
What the research shows
The first RCT, published in Archives of Pediatrics and Adolescent Medicine in 2001, enrolled 103 children aged 6-18 years with ear pain from acute otitis media. The naturopathic formula (brand name Otikon) was compared head-to-head with anesthetic ear drops. Both treatments produced statistically significant improvement in ear pain scores across the study period (P = .007), and the researchers concluded the naturopathic formula was as effective as anesthetic ear drops for managing ear pain associated with AOM.7
A larger follow-up RCT, published in Pediatrics in 2003, enrolled 171 children aged 5-18 years with ear pain and randomized them to receive naturopathic herbal extract drops (NHED), anesthetic drops, amoxicillin, or combinations. All groups showed statistically significant pain improvement over three days. The NHED group performed better than controls, though the authors noted approximately 80% of pain reduction was self-limiting and attributable to the passage of time.8
Critical caveat: Both studies used a multi-ingredient formula. Mullein was one of several botanicals; its individual contribution cannot be isolated from this trial design. The topical ear application is also pharmacologically distinct from an oral supplement. These are Tier A trials for the multi-herb topical formula, not standalone evidence for oral or topical mullein alone.

Regulatory safety review
A 2021 EFSA (European Food Safety Authority) assessment of a mullein tincture as a feed additive found it safe for short-lived food animals at the proposed maximum dose (50 mg/kg complete feed). However, the panel noted that the iridoid glycoside aucubin - present at 0.004% - has theoretical genotoxic potential, and stated "no conclusions can be drawn for long-living animals" at this time.12 While this assessment concerns animals and high-dose feed use, it provides relevant context on the aucubin concern for any long-term or high-dose human use.
